Natural Gas Burner Blast

MJ burner blast lead to the rupture of FBD.

·    Carried out Root Cause Analysis (RCA).

·    Installed preventive and mitigation barriers using Bowtie diagram.

·    Revamp the SOPs & startup checklist.

·    Trained the operating staff to reduce FLPPGs.

 

Chemical Explosion

Explosion occurred in discharge line from IEM cell.

·    Conducted HAZOP study and what if analysis (combined).

·    Root cause analysis.

·    Restructured the operational staff organogram to ensure presence of competent supervisor during silent hours.

·    Revamped the maintenance schedule to minimize EPGs.

·    Training held for maintenance dept. to create awareness about consequences of EPGs and inadequate maintenance.

 

Arc flash

Arc flash in -kv distribution transformer resulting in black out.

·    Installed the engineering controls (VCB).

·    Installed dehumidifiers.

·    Conduit the cables.

·    Arranged a basic electrical safety training.

 

Fire break out during gas cutting of rubber (EPDM) lined scrubber. Chlorine gas release from unexpected area at plant site in the drain line.
