RESPONSIBILITIES:-
OUT PATIENT PHARMACY:-
? Received prescriptions from patients, ensured prescription appropriateness, consulting with the treating doctor in case of any concerns and dispensing the same (including controlled medicines) with appropriate patient counseling.
? Maintained records of controlled drugs as per DHA guidelines i.e maintenance of FC and SC controlled books as required by DHA and maintenance of CD expiry, daily stock check, informing the chief pharmacist in case of any discrepancies.
? Expiry settlement and management, coordinating with doctors to help liquidate near expiry stock.
? Maintenance, labeling, storage of high alert medicines.
? Compiled daily statistics such as number of patients requesting prescriptions, new prescriptions received, number of refill orders, number of prescriptions not honored by the hospital and reason.
? Knowledge of PBM, E - C l a i m l i n k , Open Jet, Icare, BUPA online and Nextcare-PULSE, being the major software to obtain online approvals.
? Maintain strict control over distribution and usage of controlled drugs such as fully/semi controlled drugs with proper documentation and issuing and replacements of FC prescription books to various wards and OPDs.
? Maintained pharmaceutical inventories, storing and labeling as per DHA and JCI requirements.
? Maintained records of medication errors, incident reports related to pharmacy, near miss and adverse drug reactions. ? Organized and co-ordinated with medical representatives with regards to CPE.
? Maintained the required temperature control for the refrigerators in the pharmacy as well as room temperature and humidity and documented and filed the same.
? Maintained a record of antibiotic compliance in the hospital by the treating doctors (both prophylactic and post infection).
? Refilled ward requests for buffer stock to various wards.
IN PATIENT PHARMACY:-
? Reconciled medicines for patients upon admission and upon discharge.
? Dispensing of narcotic and controlled medicines after checking the appropriateness of the narcotic and controlled prescription and filing the same.
